PT Mitra Adiperkasa Tbk (“Company”) would like to announce the following:

1. All MAP Gift Vouchers (“Voucher”, “Vouchers”) printed since January 1st, 2019 have a validity period of two years.
2. To provide better service and improve the experience of customers when using MAP Gift Vouchers, the Company has conducted an upgrade to update the Voucher management system.
3. Vouchers published before January 1st, 2019, with serial numbers beginning with the following alphanumeric prefixes: MAPT, MAPL, M15T, M15L, M16T, M16L, M17T, M17L, M18T, M18L will be exchanged with Vouchers bearing a fixed validity date.
4. Owners of Vouchers with prefixes mentioned in point 3 above are encouraged to spend their Vouchers in stores managed by MAP Group before December 31st, 2019.
5. Owners of Vouchers with prefixes mentioned in point 3 above who do not wish to spend their Vouchers by December 31st, 2019, must exchange their Vouchers at the Company’s head office, at the below-mentioned address, no later than December 31st, 2019.
6. The exchange procedure is as follows:
   • Owner of Vouchers will have to fill out an exchange form available at the Company’s head office or can also be requested through the following e-mail: map.gv@map.co.id.
   • After filling out the exchange form, the owner of the Vouchers has to send the form, along with the vouchers to be exchanged to the Company’s head office address, using a certified courier service.
   • Company will compensate Voucher owners for the cost incurred by shipment of the returned Vouchers, the amount of which will be determined by the Company.
   • Compensation of shipping cost incurred will be decided by the Company based on area from which the Vouchers are shipped.
   • Documents/Information needed to be submitted for the exchange process:
     • A copy of the owner’s identification: KTP/SIM/Passport/KITAS
     • Active and easily contactable e-mail address and mobile phone number.
     • The postal address to send the new Vouchers to. Please submit the complete and correct address to ensure the Vouchers reach the intended recipient.
